Chronic Care Management

Chronic Care Management – defined as ongoing care for conditions that are expected to last anywhere from a year to the rest of your life – is widely recognized as a critical component of primary care, and one that has a significant impact on improving your overall well-being. If you have been previously diagnosed with a condition that requires Chronic Care Management, you can see an Amwell online provider for this type of ongoing care.

Telemedicine can be highly effective for managing chronic conditions. Many studies have shown that being able to access care online greatly increases your ability to manage your conditions. The ease of access to specialists – particularly in remote areas – means patients have greater opportunities to discuss their conditions with their provider on a regular basis. The ability to speak with a physician or specialist more frequently helps keep you on track with your care plan and gain a better understanding of your condition.

With Amwell, you can set up your free account and have a visit with trusted providers in minutes. Before your visit begins, be sure to tell your telehealth provider that you are interested in ongoing chronic care management. You should also inform them if you have any conditions other than the one for which you are seeking ongoing care, as well as any existing prescriptions or over-the-counter medications you are taking. Some medications do not react well with others, so it’s important to be open and honest with your provider to avoid prescription complications.

Your Amwell physician, at his or her discretion and in accordance with the laws of your state, may also provide you with an online prescription. With telehealth services available in all 50 states, you can have a visit and potentially receive a prescription anywhere across the country. Please be advised that some medications cannot be prescribed online, including controlled substances and extended prescriptions for birth control medications.

If you are having a telephonic or phone‑only visit, stricter prescribing regulations will apply. In certain states, a video visit may be required in order to receive a prescription.

In general, it is important for you to have the following information available for your visit:

  • Most recent lab results
  • Results of any daily testing (e.g., blood pressure, blood glucose)
  • Any medications you are taking – both prescription and over-the-counter

Many studies have shown that being able to access care online greatly increases your ability to manage your condition. The ease of access to specialists – particularly in remote areas – means patients have greater opportunities to discuss their conditions on a regular and more frequent basis. Also, the ability to speak with a physician or specialist more frequently helps you to stick with your care plan and to get a better understanding of your condition.

No. Because diagnoses of chronic conditions require specialized tests with specific equipment, they cannot diagnose your chronic condition. In order to see an Amwell physician for chronic care management, you will first need a diagnosis of that condition from a physician you have seen in person.

Currently, we can provide patients with help managing the following:

  • Diabetes
  • Headaches/Migraines
  • Birth control methods
  • Tachycardia (rapid heart rate)
  • Hypertension
  • Insomnia (cannot prescribe bridge prescriptions)
  • COPD/Asthma
  • Dermatology issues (Acne, Eczema, recurrent infections)
  • Depression/Anxiety
  • …and many more!

When an Amwell physician determines you need a change in medication, but due to either state laws or some other reason he or she prefers not to write a full prescription, the physician may furnish you with a “bridge prescription.” A bridge prescription is an order for a limited supply of medication – usually enough for 15 to 30 days – that will cover your needs until you see your primary care physician and receive a physical examination to determine if the medication is right for you and if a longer supply is warranted.
